Client medical intake forms
The following forms are designed to help us work together more easily: Please fill out the Client in-take prior to your Scheduled appointment.
If you are scheduled for an Oncology Massage please fill out the Client intake, Medical in-take part 1&2, and Oncology Massage questions form. It will be necessary to fill out these forms prior to your appointment and bring them in with you so I can prepare accordingly for your session.
This form is for all clients to complete. Please fill it out before your appointment and bring with you or you can email it to me ahead of time.
